The article in 2022 calls it an unintentional discharge (it was not). The other article describes a mistake-of-gun shooting (more likely the case).

If you don’t have serious training on how to do safe and effective simulation training, DON’T DO IT. The problem is, those who don’t have the training don’t think they need the training.
Continues to numb my mind.
