On May 23, 2018 at a range in Ohio, Massad Ayoob (one of the most famous firearms trainers) had a gun go bang when he did not intend it to do so. You can read about it here.
Rob Pincus, another famous trainer, had a great post on the subject on Facebook here. He admits to when he had his ND, and I can recall mine. It can and often does happen to people who handle firearms a lot. And layers of safety help to control the damage if they do.

The point is, mindlessness is real … and can affect anyone at any level of competence. Fortunately, no one was hurt. Much can be learned from this.
Read the posts and the blogs. Then be MINDFUL. The original 4 Safety Rules are outdated. At the RBTA, we teach: Where is your MIND?? Where is your MUZZLE?? And where is your FINGER. Finger, Muzzle, Mind. All need to be wired together with a level of deliberateness to ensure that guns go bang when we want them to in an intended direction.
